What Happened?
A 2023 data breach at Hapy Bear Surgery Center, a pediatric dental facility in Tulare, resulted in the exposure of sensitive patient information. Compromised data included names, Social Security numbers, health insurance details, and medical records.

- Up to $500 for standard out-of-pocket expenses such as bank fees, credit monitoring costs, and other related charges.
- Up to $7,500 for extraordinary losses linked to fraud or identity theft that were not reimbursed.
- An additional $50 for California residents impacted by the breach.
Beyond direct payments, all affected individuals will be eligible for two years of free credit monitoring, including dark web surveillance, real-time alerts, and identity theft insurance coverage of up to $1 million.
How to Claim
To qualify for compensation, claimants must provide proof of losses, such as bank statements, credit card statements, or phone bills.
Key deadlines:
- Claim forms must be submitted by February 4, 2025.
- The final approval hearing is scheduled for February 24, 2025.
While the total settlement amount remains undisclosed, those who believe they were affected should act quickly to secure their compensation.
Separate Settlement Available for Victims of Daily Harvest Food Poisoning
In addition to data breach settlements, consumers impacted by Daily Harvest’s French Lentil + Leek Crumbles recall may also qualify for compensation.
The crumbles, sold online and in select retail locations in Chicago and Los Angeles, contained tara flour—an ingredient allegedly linked to salmonella contamination. The FDA has reported 393 cases of illness, including 133 hospitalizations across 36 states.

- Category 1A: Up to $665 for financial damages tied to another person’s illness.
- Category 1B: Up to $1,335 for those who got sick but did not seek medical care.
- Category 2: Up to $20,000 for individuals who received treatment but were not hospitalized.
- Category 3: Up to $40,000 for those who required hospitalization.
- Category 4: Up to $173,000 for victims who had to undergo gallbladder removal surgery due to the contamination.
